29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 19 June 2019, and at times prior thereto, a worker was passing pieces of lumber up to another worker above without the use of a hard hat, exposing the worker on the ground to the hazard of being struck by falling tools, materials, or debris.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 19 June 2019, and at times prior thereto, two framers were using a nail gun and hand-held saw without the use of safety glasses, exposing the workers to the hazard of being struck in the eye(s) by flying debris or projectiles.

On or about 19 June 2019, and at times prior thereto, a framer was working on the first floor top plate without fall protection, exposing the worker to a fall hazard of approximately 10 feet 5 inches to the concrete slab.
