29 CFR 1926.453(b)(2)(v): The employer failed to ensure fall protection harness was worn and a lanyard attached to the boom or basket when working from an aerial lift.  a) At the above referenced inspection site, on or about June 20, 2019, the employer did not ensure and employee who was performing work from an aerial lift, at a height of approximately 18 feet, wore fall protection, thereby exposing the employee to fall hazards.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage and other deterioration, and defective components were not removed from service.  a) At the above referenced inspection site, on or about June 20, 2019, the employer did not ensure all fall protection equipment was inspected prior to use. At the time of inspection, fall protection equipment was found to be worn or otherwise in such disrepair that it was not suitable for use, thereby exposing the employee to fall hazards.
