Employees were exposed to electrical shock and burn hazards in that the garage door was closed on a flexible cord used for temporary power.

Employees were exposed to a fall hazard at an elevation of approximately 12-18 feet without an approved form of fall protection, to include, guardrail systems, safety net systems, or personal fall arrest systems.
