29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Jobsite located at 1111 Ridge Avenue, Philadelphia, PA: Employees conducting masonry operations on the 2nd level of a commercial construction building were not adequately trained to recognize and avoid unsafe conditions, including but not limited to; fall from height, struck-by hazards, and electrical hazards. Condition was observed on or about 19 JUN 2019.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite located at 1111 Ridge Avenue, Philadelphia, PA: Employees building an elevator tower for a commercial building were not provided adequate fall protection; thus exposing employees to fall of approximately 12 feet. Condition was observed on or about 19 JUN 2019.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: a) Jobsite located at Jobsite located at 1111 Ridge Avenue, Philadelphia, PA: Employees engaged in masonry activities were not provided fall protection training.
