29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: (a) Job Site - on or about November 22, 2019, the employer had not recorded injuries sustained by an electrician when he fell down a flight of stairs on or about June 18, 2019. The injuries consisted of fractures, loss of consciousness, and resulted in lost work days. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
