29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On or about June 24, 2019, the employer did not ensure that employees wore hard hats where they could get struck by falling metal brackets, shingles, or other material while working below the 1st story and 2nd story levels of the buildings without protective helmets while roofing or overhead work was ongoing at the Terrace at Columbine Condos located at 7520A, 7520B, 7520C, 7520D, 7520E at West Coal Mine Ave, Littleton, CO. This condition exposed employees to head injuries. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about June 24, 2019, the employer, JC Subcontractors, did not ensure that employees were protected from injury, in that 11 employees who were removing and installing shingles on residential homes and were not utilizing a fall protection system. The residential homes, located at the located at the Terrace at Columbine on 7520A, 7520B, 7520C, 7520D, and 7520E West Coal Mine Ave, Littleton, CO, had a roof height of approximately 11.9 feet for the first level and 21.9 feet for the second level. This condition exposed the 14 employees to a fall of approximately 11.9 to 21.9 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extended at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about June 24, 2019, the employer, JC Construction, exposed employees to falls from heights in that the ladder used to access the 1st story landing (11.9 feet) of the roof did not extend 3 feet above the upper working surface at the Terrace at Columbine located at 7520E at Coal Mine Ave, Littleton CO. This exposed employees to struck-by hazards due to a fall. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
