29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).  a) Facility, 415 East Broad Street, Tamaqua, PA - An employee operated the Toyota Electric Forklift Truck Model 7FBCU15 (Serial No. 60031) to transport pallets of line without having received documented/certified formal instruction, practical training, and/or an operator evaluation thereby exposing themselves and other employees to struck-by hazards, on or about June 25, 2019.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation.
