29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Employees were exposed to amputation, crushing and abrasion hazards. An employee did not utilize lockout tagout procedures when a conveyor was jammed.  On or about June 18, 2019, an employee received an injury requiring a hospitalization when their hand was caught in the belt and roller of the Pink Accumulation Line on the mezzanine.
