(a) Alvarez Roofing & Renovation, LLC at 4565 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to May 22, 2019, the exposing employer did not ensure that employees were protected from fall hazards in that employees utilized scaffolding that was not fully planked to access the roof at heights up to 30 feet above ground level. This condition exposed employees fall hazards of up to 30 feet.

This condition exposed employees fall hazards of up to 30 feet.

This condition exposed employees to fall hazards up to 38 feet.

This condition exposed an employee to hazards associated with falls when inappropriate attachment to a body harness is used.
