29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about June 27, 2019, at the pre-fabrication and machine shop areas, the employer did not have written energy control procedures for the control of hazardous energy for machines, such as, Hyundai-Kia Horizontal Boring Mill, Giddings & Lewis MC-60 Mill, OKK CNC-Vertical Mill, Johnford VMC-320 CNC-Vertical Mill, SKIQ 16 Vertical CNC-Lathe (TOS), Tarnow Lathe, Accurpress 732012 (Press Brake), Lagun Vertical Mills, Monarch Horizontal Lathe and HYDMECH M-20A Horizontal Metal Saws, among other machinery with potential to release stored energy, exposing employees to caught-in, struck-by, thermal and electrical hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) On or about June 27, 2019, at the pre-fabrication and machine shop areas, the employer had not trained authorized and affected employees in the control of hazardous energy program and procedures before operators and maintenance personal service and provided maintenance to machines such as Hyundai-Kia Horizontal Boring Mill, Giddings & Lewis MC-60 Mill, OKK CNC-Vertical Mill, Johnford VMC-320 CNC-Vertical Mill, SKIQ 16 Vertical CNC-Lathe (TOS), Tarnow Lathe, Accurpress 732012 (Press Brake), Lagun Vertical Mills, Monarch Horizontal Lathe and HYDMECH M-20A Horizontal Metal Saws. Employees were exposed to caught-in, struck-by, thermal and electrical hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 27, 2019, at the pre-fabrication and machine shop areas, the door interlock switch of the Trumatic L3030 Laser cutter was bypassed, in that adhesive tape was used to the door plate (actuator) on to the switch side, exposing employees to struck-by and thermal hazards created by the laser cutting system. d) On or about June 27, 2019, at the pre-fabrication and machine shop areas, the door interlock of the OKK CNC-Vertical Mill had not been installed, exposing employees to struck-by hazards created by rotating parts and flying metal chips. e) On or about June 27, 2019, at the pre-fabrication and machine shop areas, the door interlock of the Johnford VMC-320 CNC-Vertical Mill was not installed, exposing employees to struck-by hazards created by rotating parts and flying metal chips. f) On or about June 27, 2019, at the pre-fabrication and machine shop areas, the door interlock of the SKIQ 16 Vertical CNC-Lathe was not installed, exposing employees to struck-by hazards created by rotating parts and flying metal chips. g) On or about June 27, 2019, at the pre-fabrication and machine shop areas, the chuck of the Tarnow Lathe was not guarded, exposing employees to caught-in hazards created by rotating parts and flying metal chips.
