29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Shaver mill area, while cleaning inside shaker:  On or prior to 6/26/19, procedures were not utilized to ensure the shaker motor was isolated at all times while the shaker was being cleaned, exposing employees to hazards in the event of unplanned start-up.       Equustock LLC.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1197093, citation number 1, item number 1 and was affirmed as a final order on 1/22/18, with respect to a workplace located at 22 Ferguson Lane, Queensbury, NY 12804.
