29 CFR 1915.504(b)(8): Fire watch was not posted as defined in 29 CFR Part 1915; Subpart B as required by marine chemist, coast guard authorized person or a shipyard competent person.      On or about July 1, 2019 on the Kirby barge, employees were exposed to fire hazards while conducting welding activities inside the vessel while a fire watch was not posted as was required by the marine chemist certificate dated 5/29/19.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    On or about July 1, 2019 at the shipyard office building, an employee was exposed to a fall hazard of more than 6 feet while installing shingles on a steep roof without a fall protection system.
