29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a) On or about 06/27/19, on a roof sheeting jobsite, at a house at 716 Lafayette Ave., Buffalo, NY: A Ladder Jack Scaffold was erected on the left side of the house. Two employees were standing on the aluminum pik of that scaffold while installing roof sheeting. The pik was approx. 20 ft. long, and extended over the front support bracket by approx. 5 feet.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 06/27/19, on a roof sheeting jobsite, at a house at 716 Lafayette Ave., Buffalo, NY: A Ladder Jack Scaffold was erected on the left side of the house. Two employees were standing on the aluminum pik of that scaffold while installing roof sheeting. The employees were exposed to a fall of approx. 18 ft. because no fall protection system was being utilized.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 06/27/19, on a roof sheeting jobsite, at a house at 716 Lafayette Ave., Buffalo, NY: A Ladder Jack Scaffold was erected on the left side of the house. An extension ladder was provided for employees to access the aluminum pik on the scaffold. The side rails of that extension ladder were only approx. 32 inches above the pik.    NO ABATEMENT CERTIFICATION REQUIRED
