29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.    (a) GSI North America Inc., at 2336 Canyon Blvd Boulder, CO 80302:  On and before 7/2/19, the employer did not ensure that each employee on a walking/working surface was protected from tripping in or stepping into or through holes by covers.  Multiple holes up to approximately 6 inches in diameter were observed in the second level near the sanding/staining set up area.  Employees were cleaning and moving completed sanded/stained pieces of wood in this area.  This condition exposed employees to trip and fall hazards.

29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:  a)  GSI North America Inc. at 2336 Canyon Blvd Boulder, CO 80302:  On and before 7/2/19, the employer did not ensure that covers were secured when installed so as to prevent accidental displacement by the wind, equipment, or employees.  A 1.5 foot hole in the second floor level was loosely covered by a piece of plywood next to an air duct, and this cover was not secured to prevent displacement.  Employees were cleaning and moving product in this area.  This condition exposed employees to fall hazards of up to approximately 10 feet.
