29 CFR 1926.1203(a): Before work begins, the employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees are to enter and evaluates any permit required confined spaces:    (a) On or about July 09, 2019, employees working in the installation of underground utilities were exposed to hazards from pits, manholes, trenches and vaults when a competent person did not evaluate the potential for hazardous atmospheres prior to employee entry.    The Blevins Road Boring, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29CFR 1926.1203(a), which was contained in OSHA inspection number 1296270, citation number 01, item number 3(a) and was affirmed as a final order on 09/04/2019, with respect to a workplace located at Strom Drain Project SR 60 Tampa, FL  33619.
