29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  2262 Albright Ln. in Wheaton, Illinois 60189 - on or about July 9, 2019, employees working on Alum-A-Pole Pro-Jack pump jack systems to replace siding were exposed to fall hazards up to 16 feet because end-rails and mid rails or screens were not installed prior to starting work to prevent falls.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.  2262 Albright Ln. in Wheaton, Illinois 60189 - on or about July 9, 2019, employees working on Alum-A-Pole Pro-Jack pump jack systems were exposed to hazards associated with scaffolding instability where base plates or other devices were not placed under the loaded scaffold poles' footings to ensure firm foundation and spread the weight pressure imposed on the soil below.   There is no abatement documentation or certification required for this item.
