29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    On or about July 9, 2019, at an excavation site located on the comer of N.W. 38th and Classen Blvd. in Oklahoma City, Oklahoma where an employee was working outside the confines of a trench box, exposed to struck-by and crushed-by hazards, the employer did not ensure that employees were instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury.
