29 CFR 1926.304(d): 29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw.   On the south side of the construction site, employees operated a DeWalt skill saw with the lower guard wedged in the up position while cutting decking material.  The employees were exposed to potential amputation hazards.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.    On the south side of the construction site, employees elevated by JLG lifts to install decking material without personal fall arrest systems were exposed to potential ejection and fall hazards of approximately 25 feet.   Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.
