29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations.      a)  The employer did not ensure that the (flat) roof will support employees and equipment during roof repair that required removal of part of the deteriorated concrete of the roof slab; employee and the machine he was on broke through the roof and fell 15 feet to the 4th floor.      Location: 6500 16th Ave, Brooklyn, NY; School I.S. 227      On or about 7/5/2019      NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.856(a): Floors or working surfaces were not of sufficient strength to support the imposed load of mechanical equipment:        a)  Employer was using a ride-on floor scraper machine on the concrete roof that was weakened due to water damage; employer did not determine if the roof can support the weight of the machine; employees were exposed to structural collapse hazard.
