Located on Fields Ertel Road east of State Route 42, for a two employee flagging crew working within a multi-lane road with lane shift, multiple vehicle roadway entrances into or exits from businesses and shoulder work within the temporary traffic control zone the employer had not established a safety program procedure to ensure Crew Lead(s) technical ability to plan and design in detail necessary to follow principles of part 6 MUTCD 2009 Temporary Traffic Control Plans to set up a more complex temporary traffic control zone(s) in that the company safety program lacks at the minimum the specific elements of MUTCD 2009 edition Temporary Traffic Control Plans and to include 6G and 6H 1-46 diagrams with training procedures on 6H diagrams to include the usage of multiple combinations of 6H diagrams

Located on Fields Ertel Road east of State Route 42, for a two employee flagging crew working within a multi-lane road with a lane shift, multiple vehicle roadway entrances into or exits from businesses and shoulder work within the temporary traffic control zone the employer had not established safety Crew Lead training to ensure a Crew Leads technical ability to plan and design in detail necessary follow principles of part 6 MUTCD 2009 Temporary Traffic Control Plans to set up a more complex temporary traffic control zone in that the Crew Lead specific training lacks review at the minimum the specifics of MUTCD 2009 edition 6G and 6H 1-46 diagrams and training on 6H diagrams to identify the usage of multiple combinations of 6H diagrams

29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers, shall conform to Part 6 of the MUTCD (incorporated by reference, see �1926.6). Section Located on Fields Ertel Road east of State Route 42 there were two flagging crew employees (Crew Lead and Passenger) observed directing traffic into and out of an entrances roadway from a businesses while being located at times on the center dividing road lane line marking of the multi-lane roadway in a temporary traffic control zone which comprised a multi-lane road with a lane shift, multiple vehicle entrances into or exits from a businesses and shoulder work within the temporary traffic control zone while not controlling all road users in that hand signals only were being utilized without the use of stop/slow paddle devices to obtain road users attention and provide positive direction exposing the employees to being struck by vehicular traffic.
