29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Worksite located at 151 Mullen Road, Belton, Missouri- Employer exposed himself as well as his employee to fall hazards while being elevated in a work platform by an operator that had not completed proper training and evaluation of his training in order to operate the machine in a safe manner.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    Worksite located at 151 N. Mullen Road, Belton, Missouri- Employees were exposed to a fall hazard while working in an elevated work platform. The platform was not properly supporting the employees as they were moved horizontally into their work positions.    The Jose Mora Stucco company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(c)(2)(v) which was contained in OSHA inspection number 989087, citation number 1, item number 2 and was affirmed as a final order on May 2, 2015, with respect to a workplace located at 6633 W. Kellogg, Wichita, KS 67207.
