29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  (a): Exterior, adjacent to Grand Ave - Employees working from aerial lifts did not have lanyards attached to the boom or basket. Three employees were exposed to fall hazards of approximately 30 feet while wearing personal fall arrest harnesses that were not attached to the boom or basket while working on aerial lifts, on or about July 11, 2019.   There is no abatement certification or documentation required for this item.
