On or about July 11, 2019, employees were using two ladders to gain access and exit from the roof.  One was a 12-foot multi purpose ladder that was only extended one rung above the landing and the 18-foot multi purpose ladder was only extended 1 1/2 rungs above the landing, exposing employees to fall hazards.
