29 CFR 1926.55(a): Exposure of employees to inhalation, ingestion, skin absorption, or contact with any material or substance at a concentration above those specified in the "Threshold Limit Values of Airborne Contaminants for 1970" of the American Conference of Governmental Industrial Hygienists, was not avoided:      At 751 Gravois Bluffs Blvd in Fenton, MO 63026 - On or about 07/16/2019, employees were exposed to Carbon Monoxide (CO) above 50 PPM and up to 82 PPM while operating gas powered equipment inside a commercial building to cut out and remove portions of the concrete slab for new utility installations, and these inhalation hazards were not avoided.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    At 751 Gravois Bluffs Blvd in Fenton, MO 63026 - On or about 07/17/2019, employees were exposed to flying particles, such as concrete chips, while operating a propane "Core Cut" (cc 1800 / Serial No. 151123) walk-behind concrete saw and the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.
