The ground pin was removed from the extension cord, on or about, July 16, 2019.  There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a): Exterior Roof- Four employees engaged in residential construction 6 feet or more above lower levels were not protected by any method of fall protection. The employees were exposed to fall hazards of approximately 25 feet while working on the roof, on or about July 16, 2019.  There is no abatement certification or documentation required for this item.   Alpine Construction & Interior Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1232929, citation number 2, item number 1 and was affirmed as a final order on July 11th, 2017, with respect to a workplace located at Van Wagoner Drive, Englewood Cliffs, NJ.  Alpine Construction & Interior Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1114344, citation number 1, item number 2 and was affirmed as a final order on February 18th, 2016, with respect to a workplace located at 133 Reldyes Avenue in Leonia, NJ.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a): Exterior Roof- Portable ladders used for access by employees to an upper landing surface did not have the ladder rails extend at least 3 feet above the upper landing surface. Employees were accessing a roof by using a portable extension ladder that did not have rails extend at least 3 feet above the upper landing surface, on or about, July 16, 2019.  There is no abatement certification or documentation required for this item.  Alpine Construction & Interior Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1232929, citation number 1, item number 1 and was affirmed as a final order on July 11th, 2017, with respect to a workplace located at Van Wagoner Drive, Englewood Cliffs, NJ.
