29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation;  A) On or about July 17, 2019 at 57-29 49th Street, Maspeth, NY 11378  Employees required to use hazardous chemicals, such as but not limited to, propane gas while changing propane tanks on a powered industrial truck (i.e. forklift) were not provided with appropriate eye protection.   ABATEMENT CERTIFICATION IS NOT REQUIRED

29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.  A.) On or about Wednesday, July 17, 2019 at 57-29 49th Street, Maspeth, NY 11378  The employer failed to record an injury involving days away from work for calendar year 2019.  The workplace injury occurred on Thursday, June 6, 2019 and resulted in four (4) lost work days.  ABATEMENT CERTIFICATION IS NOT REQUIRED
