29 CFR 1910.147(c)(4)(ii): Energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be used for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the elements listed in this paragraph.  a) TWB III, Gate #80 (Back gate were scrap is removed): On July 18, 2019 and at times prior, the employer exposed employees to potential hazardous energy sources such as, but not limited to electrical, hydraulic, gravity, and kinetic, in that the written procedures were not updated to include changes to control of hazardous energy once gate 80 of TWB III could no longer be locked out by use of the control panel and/or the disconnect located on the gate.  Employees were able to access the area through the gate without the system being locked out.
