29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: a) On or about July 15, 2019, employees conducting re-roofing activities using a Werner 16 foot extension ladder was not used in an adequate angle in that it was extended 12 feet and the distance between the foot of the ladder and the wall was 6.5 feet from the wall exposing them to a fall hazard.
