29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  a) Poly Pak Industries., Elizabeth, NJ/ Warehouse- The employer failed to certify that each operator had been trained and evaluated to operate a PIT. The employer failed to provide documentation which included the name of the operator, the date of evaluation and the identity of the person performing the training.   Observed on or about 07/12/2019.
