29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    The employer did not provide an adequate Lockout/Tagout procedure for the changing of an air compressor pressure switch.    AVD Changed due to Informal Settlement Agreement:    The employer did not develop a written Lockout/Tagout procedure for the Speedair (model: 35Wc55) air compressor.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:    On or about July 19, 2019 an employee conducted maintenance on a Speedaire air compressor at the Trinity-Haynes A1-7H production well when an explosion occurred.   The employer had not notified the site owner or exchanged their lockout /tagout procedures.
