At the worksite On or about and times prior to July 11, 2019, a Lull Highlander forklift (model: 644B-42) was operated with an inoperable horn, a damaged left fork, and a damaged carriage, thus exposing employees to struck-by hazards.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a). Top of the bleachers On or about and times prior to July 11, 2019, employees were welding angle iron and metal vertical supports while working from the roof of metal containers and from an aluminum platform located behind the containers without the use of a fall protection system, thus exposing employees to a fall hazard up to approximately 28 feet.
