The tools were connected to an extension cord, which was routed to an outlet device in the house backyard. The extension cord was missing a ground pin. On or about 07/16/19.

The tools were connected to an extension cord, which was routed to an outlet device in the house backyard and was not provided strain relief to its outlet. On or about 07/16/19.
