29 CFR 1926.651(b)(2): Utility companies or owners were not contacted within established or customary local response times, advised of the proposed work, and asked to establish the location of the utility underground installations prior to the start of work.  a) JTS Excavating, LLC Located at 284 Hawking Drive GALENA, OH, 43021:  On and preceding July 24, 2019, the employer did not contact the necessary utility companies to establish the location of the utility underground installations prior to the start of work.  The company was in the beginning stages of digging to install a storm drain system for a new residential development when an underground gas line was struck the day prior to the inspection.  The following condition exposed employees to fire and explosion hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) JTS Excavating, LLC Located at 284 Hawking Drive GALENA, OH, 43021:  On and preceding July 24, 2019, the employer did not ensure that a stairway, ladder, ramp or other safe means of egress was located in trench for employees to safely exit the trench.  The company was installing a storm drain in a new residential development which, at the time of the inspection was 6 feet deep trench with one employee working inside.  The following condition exposed employees to struck-by and caught-in hazards.

The company was installing a storm drain in a new residential development which, at the time of the inspection was 6 feet deep trench with vertical walls and one employee working inside.  The following condition exposed employees to struck-by and caught-in hazards.
