At residential construction site located at 2419 Woodward Hills, lot 172 in Clermont, FL, an employee was exposed to fall and struck-by hazards when helping lift a 24' x 16" floor joist onto a 9-foot wall while standing on the top of a 6-foot A-Frame Aluminum ladder.  Employee fell off the ladder and struck by the joist, on or about July 18, 2019.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  At residential construction site located at 2419 Woodward Hills, lot 172 in Clermont, FL,  on or about July 18, 2019:   a) Employee was exposed to fall and struck-by hazards when helping lift a 24' x 16" floor joist onto a 9-foot wall while standing on the top of a 6-foot A-Frame Aluminum ladder and not provided with a training program.    b) Employees were exposed to fall and struck-by hazards when using a 6-foot A-Frame Aluminum ladder to access 9-foot concrete walls to install 24' x 16" floor joists and not provided with a training program.
