29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Jose Dolores Rovles located at 4979 Vicksburg Lane, Hilliard, OH, 43026:  On and preceding July 25, 2019, employees were removing shingles, preparing for a re-roof on the roof of a two story residential home. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jose Dolores Rovles located at 4979 Vicksburg Lane, Hilliard, OH, 43026:  On and preceding July 25, 2019, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were conducting removing shingles on a roof that varied from 10-18 feet with the employees working at different levels. The employees were not utilizing proper head or fall protection which exposed the them to falls up to 18 feet and struck by hazards to the head.

Roofers were conducting removing shingles on a roof that varied from 10-18 feet with the employees working at different levels. The employees were not utilizing proper head protection which exposed the them to struck by hazards to the head.
