29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  On or about July 25, 2019, the workplace assessment, designed to identify personal protective equipment (PPE) needed by employees to prevent injury, had not been performed.   NOTE:  The assessment shall be written, titled, dated and certified by signature.  The required documented training shall include the following:    1)     When PPE is necessary;  2)     What PPE is necessary;  3)     Limitations of the PPE;  4)     How to properly wear and use the PPE, and 5)     Inspection and care of the PPE

29 CFR 1910.134(c)(2)(ii):  The employer did not establish and implement elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user:  On or about July 25, 2019, the employer had not established the program as required for the workers' voluntary use of respiratory protection as they were provided half-mask negative air respirators for use in the repair shop area.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:  On or about July 25, 2019, the employer had not compiled and made available to the workers a written hazard communication program for such hazardous materials/chemicals as, but not limited to the stains, solvents, and other chemicals in use in the furniture repair shop.
