29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a.  On or about July 25, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a.  On or about July 25, 2019, at the above addressed jobsite, employees performing carpentry operations from the roof were exposed to falls, of eleven (11) feet two (2) inches, created by the presence of unprotected sides.

On or about July  25, 2019, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
