29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about July 26, 2019, employees were exposed to a struck by hazard.  The employees were performing roofing activities on a commercial building located at 735 North Revere Road, Akron Ohio 44333.  The employees were using air powered nail guns without wearing protective safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about July 26, 2019, employees were exposed to a fall of approximately 15 feet.  The employees were performing roofing activities on a commercial building located 735 North Revere Road, Akron Ohio 44333.  There were three employees working on the roof without using any fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 26, 2019, the employer failed to ensure that an aluminum ladder being used to access the roof, was extended at least 3 feet above the upper landing surface on a commercial building located at 735 North Revere Road, Akron Ohio 44333. The approximate height of the upper landing access point was 11 feet.
