29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) For employees working at 2691 NE Pine Island Road in Cape Coral, as observed on or about July 25, 2019. Employees were observed installing roofing material on a 6:12 pitch commercial roof without the use of a fall protection system. Employees were exposed to a fall height of approximately 18 feet 6 inches.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) For employees working at 2691 NE Pine Island Road in Cape Coral, as observed on or about July 25, 2019. Employees were installing roofing material on a 6:12 pitch commercial roof. The employer failed to maintain a certification of record of fall protection training.
