29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another. a) On or about 07/24/19, on a roofing job site, at a house at 517 Lafayette Ave., Buffalo, NY: A ladder jack scaffold consisting of 3 support ladders and two approx. 20 ft. long aluminum piks was erected on the front side of the house. The left pik overlapped (bridged) the right pik by about 2 ft. at the middle support ladder. Employees go on the piks to access the roof and for other work. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 07/24/19, on a roofing job site, at a house at 517 Lafayette Ave., Buffalo, NY: Three employees engaged in roofing work on the steep roof on the front side of the house were exposed to a fall of approx. 20 ft. because fall protection was not being utilized. Pitch of the roof was approx. 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED

The left pik extended over its support bracket on the left support ladder by about 4 feet. Employees go on the piks to access the roof and for other work. NO ABATEMENT CERTIFICATION REQUIRED
