29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  a). Roof of the building - On or about and times prior to July 19, 2019, employees worked from the roof while removing and replacing roof tiles without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 27 feet.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  a). Roof of the building - On or about and times prior to July 19, 2019, the employer did not ensure employees were trained by a competent person on the use and operation of warning line and safety monitoring systems for employees that worked from the roof while removing and replacing roof tiles, thus exposing employees to a fall hazard of approximately 27 feet.
