29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a)    26 S Main St, Medford, east side of bldg: Employees without a personal fall arrest system or any other means of fall protection, were working from two ladder jack scaffold platforms at heights greater than 10' and up to approximately 11' above the sidewalk, while installing shingles on the approximately 3'2" deep overhang running the length of the front of the building, on or about 07/259/2019;    b)  Northeast corner of building, 26 S Main St, Medford, east side of bldg: An employee without any fall protection, approximately 11 feet above ground level and working from the exterior of the scissor lift with AMERICAN EQUIPMENT painted on the outside structural member for the platform, was either sitting on the top rail or standing on the mid-rail, while installing flashing or shingles, on or about 07/29/2019.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
