29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure:  a) 3022 Titan St, Philadelphia, PA - The employer allowed employees to work approximately 12 to 30 feet above the ground without a fall protection system in place thus exposing employees to fall hazards. This condition was observed on or about July 11, 2019.
