29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.     On east side of the construction site, employees worked from a platform made of sheetrock material that was elevated by a sky jack forklift.  The work platform was not an approved work platform by the manufacturer.  The employees were exposed to unprotected fall, struck-by and crushed-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On west side of the construction site, employees performing framing activities without fall protection were exposed to unprotected fall hazards of approximately 12 feet.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.     On west side of the construction site, the employer did not provide an extension ladder where employees could safely access and egress the buildings upper landings.  The employees were exposed to potential fall hazards.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   On or before July 30, 2019, and at times prior thereto, the employer did not provide hazard recognition training or avaoidance and exposure of hazards such as but not limited to:  a. employees performing framing activities on a roof without fall protection were exposed to unprotected fall hazards of approximately 12 feet,  b. employees were elevated on an un-approved work platform work and to gain access to the upper roof areas exposing them to fall hazards.
