29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about July 30, 2019, and times prior thereto in the manual lathe machine shop:  a. In the machine manual lathe machine Yamazaki Machinery Works # 8, an employee was exposed to a caught-in hazard while sanding rotating parts.  b. In the machine vertical milling machine # 22, where the employees were exposed to flying metal shaving when operating the machine that was not guarded.  c. In the machine vertical milling # 23, where the employees were exposed to flying metal shaving when operating the machine that was not guarded.

A bench grinder brand Central Machinery, 6 in, 1/2 hp, located in the manual lathe shop, south wall, was used without work rests adjusted to the wheels with a maximum opening of one-eighth inch exposing the employees to a struck-by hazard.  b. A bench grinder 6 in, 1/2 hp, located in the saw shop, east wall, was used without a work rest adjusted to the wheel with a maximum opening of one-eighth inch exposing the employees to a struck-by hazard.

A bench grinder brand Central Machinery, 6 in, 1/2 hp, located in the manual lathe shop, south wall, was used without a tongue guard on the wheel exposing the employees to a struck-by hazard.    b. A bench grinder 6 in, 1/2 hp, located in the saw shop, east wall, was used without a tongue guard on the wheel exposing the employees to a struck-by hazard.
