29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    (a) On or about July 30, 2019, and at times prior, Dynamyc Construction, LLC, as the exposing employer, did not ensure an employee, working from an aerial lift, attached the lanyard to the basket while installing siding, exposing the employee to fall hazards of approximately 47 feet.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
