29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose:    1) On July 31, 2019, and at prior times, Pine Valley Electric, Inc., as the creating employer, did not ensure that employees of Two22 Brew, LLC were protected from exposure to electrical hazards, in that a junction box designed to be installed on a wall as a part of a permanent electrical wiring was used as a part of a flexible electrical cord and plug system.  The box was not adequately supported which could cause damage to knockout covers.  The wiring was not adequately supported and the box was not approved for use as a flexible electrical cord and plug system, which could transmit strain to terminal connections.
