29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about August 1, 2019, and at times prior thereto employees was exposed to struck-by hazards due to not wearing eye protection while utilizing a pneumatic nail gun to secure decking materials on the roof.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   On or about August 1, 2019 and at time prior thereto, employees re-roofing a building were elevated and lifted onto and off the roof by a forklift and a wooden pallet platform that was not approved or designed for use by the forklift manufacturer exposing the employee's to fall hazards of approximately 30 feet.
