OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught-in between the trench box spreader and the excavator:    On or about 7/24/2019, at 6411 SW 20th Street Miramar, FL; the employer did not ensure the operator maintained a safe operating distance between the equipment and employee.
