29 CFR 1910.269(d)(6)(vii): Before starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergizing of the machine or equipment had been accomplished:  a) Busby Substation: On August 2, 2019 and at times prior, employees were exposed to shock hazards of approximately 7200 volts of A/C electricity while clearing debris from Big Horn County Electric's equipment.
